a player from the reserve list who replaces a player who's been knocked out of a full tourney early on

usually permitted in the first two levels of a live comp. Not thereafter.

and sometimes referring to players who aren't replacements for knocked out players but who fill extra tables added after the start of a comp which was more popular than expected

as with so many other things it's venue dependent.

I believe at grosvenors you get a full stack but have to put in 1xbb and 1xsb as dead money into the first pot. some venues work out how many blinds you've missed and reduce your stack by that much which never goes into play and others give you the lot

No.  If only 5 people get knocked out in the first two levels (the most common rule for alternates in my experience), then only 5 alternates will get into the tournament, even if there are a hundred on the list.

Of course, if the term is used for additional players joining additional tables (as gatso mentioned), then I see no reason why a lot (if not all) shouldn't play.

as Rich says it's usually only for the first 2 levels (I've never once seen it beyond that point). anyone not given a seat by the cut off time cannot play.

as an example today's EPT has a reserve list of over 50. there is no way that everone on that list will get in

Gala in Leeds started imposing a 20% penalty for latecomers but dropped it after a couple of weeks as it seems it may have been illegal.
