If you go to this address, type in your details to get a copy of the last one you filled in. since then it has changed and now cost $14 however the ESTA lasts 2 years and does not need renewing even though no it needs paying for if you havent already been issued and approved. I flew into New York last week on the same ESTA i filled out for free last summer without any problems The visa you fill out will be passed to you during your flight on the plane Hope this has helped. All my family that went in 2009 have had 3 or 4 emails saying it runs out in a few more weeks
