Think this would take a while for ever so....

Bobby Zamora, then of West Ham, who scored, assisted and was then sent off with the Hammers leading 3-2 at home to Fulham in January 2007.
Marc-Antoine Fortune helped West Brom to a 3-0 win at Southampton in 2013 with a goal and assist, but was given a straight red along with Gaston Ramirez with the score at 2-0 in the 70th minute.

Coventry aren't the lowest. It's Wimbledon if you see us as the continuation of WFC, and Portsmouth if you don't.
