whats this form that you need to fill in if you want to go to the usa

seemingly as of today if you dont fill it in before you go you will get sent back

Only Green Visa Waiver form and customs form as far as I know from a couple of months ago..........

Got back on saturday, nothing different then, but yes from the 12th you have to complete more forms...

It's basically the Visa Waiver form, but an electronic version.  You need to fill it out in advance and once approved (pretty much instant in my case) it's good for 2 years.

I still had to do the Visa Waiver and Customs forms on the flight in late December but it'll ultimately replace the Waiver form (probably not the Customs one though).

I got the website link via BA's booking page.  Can't remember it off the top of my head.

All fairly straightforward stuff though.

You have to fill out the online form at least 72 hours prior to travel.  Once approved, it's good for 2 years and multiple trips.

Lie about it on the forms they wont find out unless you tell them.
