If you want I will email our forms from work, with all the explanatory notes on how to score, when they should be done etc. Will have to wait till monday though.

I work in local govenment so they will be prery much standard appraisal forms..

Does 360 feeback work for a force of one? 

Anytime I have been involved in 360 feedback its involved at least groups of 4 people.

You should devise your own appraisal form based on the skills relevent to your own business and ask you employee to write in each section how they performed against any set targets you may have set for them, also ask them to identify areas for development.

In turn after reading what they have put together it may be worth you writing up your thoughts and then have a discussion on it. 

I think staff appraisal is more valuable in smaller companies than it is with bigger companies.  What you find with larger organisations is these areas normally become a paper excercise and dependent on the frequency of the appraisals it becomes de-valued and more often than not a copy and paste job.
