Hi - I want to introduce staff appraisals for my very large workforce of 1.....
Have no idea what to put on the form, I want them to fill one out and I do the same and then we go through them together.
Does anyone have any experiences of this? Is there a source for forms (or one I can steal from somewhere?)

Google for forms for a process called "360 degree feedback"
It was introduced by a lot of HR departments in the last 15 years, sounds like what you need...ie they feedback on you, you feedback on them
More useful than a one way convo....

firstly, just set out what it is your company does on paper. Then set out what you want the employee to do to contribute to the overall success of the company. Then you basically score them and analyse areas for improvement. ie if they have been rude to a customer then thats an area for improvement.
ill see if i can find any tools online
