You could have perhaps raised more preflop.
Other than that your only mistake in the hand was not raising him on the flop.

Its hard to give a proper opinion without knowing how the table view you wht kind of image do you have and perhaps more importantly any reads on the villian?
But as you have laid it out I would have probably tried to get a bit more in the middle pre-flop and definently re-raised on the flop.
Come the turn (depending on reads) one of the hands you have to worry about is 3 Queens as given the action its a plausible holding.

I think my biggest misktake was not raising enough preflop. I got caught in between trying to keep the reraiser in and making sure the other guy folded, therefore resulting in my raise not being able to define my opponent's hand enough. (ie, makes J-J and T-T a possible holding)
If I know he only has Q-Q, K-K, or A-K, then I'm a happy man.
With regards to the flop, I was happy to flat call. Considering the small selection of hands he could have (as shown above), I'm not scared of the turn. If he has Queens and outdraws me, then so be it, it's a two outer.

I'd agree with that...a larger pre-flop raise would have been nice. I also would have re-raised on the flop as there's a flush draw out there....but that's about it. Yeah you got lucky on the river but then again he got lucky on the flop....

I am loving the flat call on the flop. Raising just gives him a chance to bin QQ/TT.
If you flat call he is almost always firing the turn, then you can raise and he is far more commited.
A larger re-raise pre though is probably the correct thing to do, or flat call.

I'm not worries about the flush draw. The only possible flushing hand is A-K of diamonds, which is pretty unlikely, and even then he's not going to throw it away to my reraise on the flop, so I might as well let him in cheap and give him a chance to bluff the turn.
Well, that was the thought process anyhow.
