Not sure but Dean windass was given 3 red cards in 1 game before

On 9 November 1997, during a league game with Dundee United, Windass was shown three red cards - once for foul play (having previously been booked); another for verbally abusing the referee; and a third for taking out his frustration on a corner flag as he left the field - for which he received a six match ban

This is standard on Football Manager! If you appeal against a refereeing decision and they see no grounds for you to do so then they add a game on making it 4 games instead of 3!

at first I thought it was a bit harsh punishing the player for the club appealing but now I like it as we know cash fines are no deterrent to clubs doing anything, this might be

Except in this case, where the red was clearly wrong, they would still have appealed.

Has happened recently, Boro had a game added to one player's ban for a 'frivilous appeal', again the red was harsh if not just plain wrong.

Contrast that with John Terrible's  over-turned red for his last man professional foul last season, and it seems there is one rule for the top teams, and another for the rest.

Except in this case, where the red was clearly wrong, they would still have appealed.

The idea in principle is great, however in practice it's been a bloody disaster, calling 'frivilous appeal' on perfectly goods appeals, yet granting appeals on very very dodgy grounds.
