I think it's a combination of both Ralph.
Bad general standard of play + rake being hard to beat.
Mitchell and Keith Johnson gotta be the biggest winners?

Nevermind DTD, i'd imagine you could pick any venue around the world (bricks and mortar and online) and the ratio of winning players to losing players will be very similar (not inc rakeback). Such is the intricate nature of the poker economy.
