The clue (1 down) is "secretly getting uplift from bras or bustiers", which suggests "Cup Hook".

The carthorse clue would start with the fifth letter ("O") of that.

Looks like Posh Bloke, The Milkman & eventually Tighty, were all correct.

Pretty sure the answer to 1 down is Sub Rosa, but the O bit still applies

More likely SUB ROSA. Bold bit backwards (as it's a down clue, it will read upwards = uplift)
