I'd definitely decline both pieces of information

To those that want to know so that they can do stuff before their date comes, why not just work on doing the stuff that you want to do now?

Haha well then I guess I'd go for the date so I'd be able to learn to read by the time.

The date has the least downside I think, even if its today or tomorrow you can still do some fun stuff but if you found out that it'd be a slow degenerative condition I think I'd be pretty jumpy on the lookout for early symptoms etc, would be hard to not think about

You could jump from the plane if you knew you were destined to die in a train crash too, as long as there were no trains in your landing area that is.

I'd be happy to be still aware enough to be able to make decisions such as this right up until the date of my death.  I think the date is definitely most useful.
